The 'bad' endings come from the MCS-X dosing, so just do not do that. She is kinda an escort either way earlier in the story, but you never see it. When you find out, you get the option to be one also. It's not as bad as it sounds, really. Enjoy!
No it's not set in stone. The two you listed are as Zephea says, bad ends... It's a highly experimental version of the MCS serums after all.
There are other options depending on what serum path(s) you took though. That's why saving is a must when presented with options to choose from.

From what I read she's an escort pretty much no matter what at the start, so guess your locked into a bad ending from the start. unless you want share how to avoid that then it be appreciated. if not stick to what I said either route isn't my thing.

Her being an escort exists so that the MC has it as an option once it's triggered.
Basically, she's been doing it as soon as you start the game.
Depending on the serum choices you make she can become exclusive to the MC.
I'm not going to share anything else beyond that.

it's a matter of perspective it's fine, me not being a fan of the routes and content isn't a big deal. I asked got an answer and move on, thank you for the replies I appreciate it.
I looked at the game again after two years and played through the game mechanics are nice with drug control and such.
But the story really? The player is forced to give up control to non-player characters to survive chapter 5 ...
No thanks, I'd rather not play through the story again with a different approach.
From a technical point of view, it doesn't make sense to target only one group when you can expand the story to appeal to several target groups with a little extra effort?

I mean with Breaking Bad and Sons of Anarchy you ask yourself with every episode can it get worse, yes it can. But if they had just let the main characters die at the end without doing a good deed, you would never watch the series again.

The current ending of Chapter 4 is meant to be a Cliffhanger. Chapter 5 will settle what actually happened but that will be the final thing Westane adds. It's by no means the actual ending of the game and in fact, The Traitor can and will change depending on the choices you made.

Only Dom John has straight up NTR where he steals every girl from you. The rest of the game is a bit iffy in terms of what you would consider NTR or not

We'll find out in 2 years when the Epilogue is added. So far the only consequence is you can never change your gender again. For now there is no way to ever see a baby bump or give birth. Everything pregnancy related is Epilogue ONLY
This may be a dumb question as I haven't played this in awhile but did they remove the Dakota "Bitch in Heat" route?
